ACT is an intensive integrated approach that provides community based mental health treatment, rather than a more restrictive residential or hospital setting. In essence ACT offers treatment in the “real world” and does so using an interdisciplinary team of professionals. The goals of ACT include reducing homelessness, unnecessary hospital stays, improving treatment adherence and improving the quality of life for our individuals served.

Individuals 18 years or older, who hold a primary diagnosis of serious mental illness to include: Schizophrenia, bipolar Disorder, schizo-affective disorder, delusional disorder, Psychotic Disorder NOS) with high service needs of one or more of the following: high use of acute psychiatric hospitals (multiple admissions in the last two years, at least one long term stay of 30 days in a psychiatric hospital in the last two years,  high use of behavioral health crisis services (4 or more interventions in the last 12 months), intractable severe psychiatric symptoms, co-occurring mental health and substance use disorder, high risk or recent history of criminal justice involvement as result of the individual’s mental health disorder, significant difficulty meeting basic survival needs (homeless or imminent risk of homelessness as result of the individual’s mental health disorder), risk of requiring a residential or institutional placement if more intensive services are not available and an inability to participate in traditional office-based outpatient treatment.

Mental health skill-building services encompass a range of supportive interventions designed to enhance an individual’s coping mechanisms, resilience, and overall mental well-being. These services aim to equip individuals with practical tools and strategies to manage stress, emotions, and challenges effectively. The goal is to empower individuals with the skills and resources they need to navigate life’s challenges and maintain optimal mental health.

Individuals 18 years or older, who hold a primary diagnosis of serious mental illness to include: Schizophrenia or other psychotic disorders, major depressive disorder, recurrent bipolar I or bipolar II or other serious mental health disorders with a history of psychiatric hospitalizations, either residential or nonresidential crisis stabilization, intensive or assertive community treatment, placement in a psychiatric residential treatment facility (PRTF) as a result of decompensation related to the individual’s serious mental illness; or a temporary detention order (TDO) evaluation and with who have received a prescription for antipsychotic, mood stabilizing, or antidepressant medications within the 12 months prior admission to services, or documentation from a physician or other practitioners licensed to prescribe medication that indicates that the use of these medications are medically contraindicated for the individual.
