The Psychiatric Care Provider serves as a medical director of the Assertive Community Treatment (ACT) team taking the lead in all psychiatric treatment and monitoring all other health conditions and medications.

In collaboration with the psychiatric care provider, the RN is responsible for providing medication management and psychiatric nursing services to individuals served by the Assertive Community Treatment (ACT) team. Coordinates services and/or needed resources to support individuals to maintain or return to community living. Conducts nursing assessments; making referrals to community physicians; and providing a range of treatment, education, rehabilitation and support services.

In collaboration with the psychiatric care provider, the LPN is responsible for providing medication management and psychiatric nursing services to individuals served by the Assertive Community Treatment (ACT) team. Coordinates services and/or needed resources to support individuals to maintain or return to community living by making referrals to community physicians; and providing a range of treatment, education, rehabilitation and support services.

An ACT team member who provides integrated treatment for co-occurring disorders to individuals who have a dual diagnosis of persistent serious mental illness and substance abuse. Responsible for carrying out rehabilitation, clinical and support functions including but not limited to linking and referral, education, individual and family support counseling, developing ISPs, mental health support services, symptom management and crisis intervention.

Works as part of the Assertive Community Treatment team to assure services and resources are coordinated among providers as needed to help individuals served
live and function optimally in the community. Responsible for carrying out rehabilitation and support functions including but not limited to service assessments, individual and group therapy coordination, developing ISPs, psychoeducation, education and skill building for recovery and mental health stability, community resource linking and coordination, mental health support services, symptom management and crisis intervention.

An ACT Team member who provides supported employment and education services including engagement, vocational assessments, job development, job placement and/or attending school or classes, job coaching and/or academic support, and benefits counseling. Responsible for carrying out rehabilitation and support functions including but not limited to service coordination, developing ISPs, education and skill building for recovery and mental health stability, community resource linking, coordination, support, and training for reaching employment or academic goals.

Self-identifies as an individual with a serious mental illness who is currently or formerly a recipient of mental health services, and is in the process of their own recovery. Functions as a fully integrated team member to provide personal insight about the recovery process and what it is like to experience recovery from a serious mental illness. Provides input and support about the recovery process, symptom management and skills needed to maintain progress towards recovery. Provides peer counseling and support to individuals served, families, team staff and acts as liaison with community resources. Provides services with intent to maximize individuals’ quality of life, daily functioning and ongoing prevention of hospitalization for mental health concerns. Works to develop trusting relationships with individuals served while providing peer counseling and consultation to individual clients and families.
